Abstract

Qu'ils soient évoqués spontanément par le patient ou recherchés par le médecin, les troubles du sommeil sont très répandus. Il s'agit parfois d'un symptôme de mal-être, mais aussi souvent d'une maladie à part entière comme le syndrome d'apnées du sommeil, à titre d'exemple. C'est pourquoi la médecine du sommeil est une discipline transversale proche de la médecine interne. En effet, pendant le sommeil, le temps se réorganise, le psychisme se reconstruit, la personne recouvre ses forces physiques et l'organisme sécrète un certain nombre d'hormones. Dans la solitude de la nuit l'inconscient cherche à se libérer, les désirs s'expriment dans des rêves ou se refrènent en de terrifiants cauchemars. Parfois, le sommeil se refuse sans que rien n'y fasse. Alors la plainte parvient aux oreilles du médecin. La tentation est alors grande de prescrire un somnifère ou un « remontant ». Pourtant, bien souvent, les troubles du sommeil sont la traduction d'une maladie sousjacente, d'une souffrance ou encore l'un des effets secondaires d'une thérapeutique mise en place pour une autre pathologie. Cet ouvrage se veut volontairement pratique avec des exemples concrets de façon à constituer un outil avec nombre d'adresses utiles et d'explications en annexe pour mieux cerner ces maux, mieux les situer au sein d'une nosologie et donc mieux les traiter.

Abstract

'Rare Diseases Epidemiology' offers numerous approaches to increase the knowledge base of rare diseases and conditions and to facilitate the development and dissemination of interventions for the prevention, diagnosis, or treatment of over 6500 diseases and conditions. The goals of epidemiology are not restricted merely to providing numerical data on the prevalence or incidence of diseases in a limited or general population. Analyses of epidemiological data gathered from appropriately designed and conducted studies are required to establish public health policies and priorities in all nations. Information gained from these studies lead to a better understanding of the etiology and the impact of genetic or environmental factors on the occurrence and outcome of these disorders. The term Rare Diseases includes both acquired and inherited disorders. 'Rare Diseases Epidemiology' provides methods and approaches from the collective experiences of established research investigators who address these significant issues of the development of patient registries; the collection, storage and selected distribution of bio-specimens from bio-banking activities; the validation and utilization of genetic testing and newborn screening procedures; the presentation of issues related to the importance of case reports to increase knowledge of rare diseases; the challenges and models for population-based surveillance studies for rare congenital and inherited disorders; the statistical methods for the geographical analyses of rare diseases; the value and need for clinical trials and comparative effective studies; and meeting the requirements of regulatory agencies. Economic, societal, and ethical concerns are presented as patients and families encounter difficulties obtaining the correct diagnosis, gaining access to treatments, and receiving coverage or reimbursement for approved interventions, and for developing a public understanding of the costs to patients and their families and the burden of illness affecting the quality of life of millions of patients with rare diseases and conditions. Framework programs for rare diseases research as developed by the European Union and the value of national plans for individual member countries are discussed and represent the public commitment to patients with rare diseases. Likewise, similar programs have been implemented in the USA as a result of the Orphan Drug Act to address the needs of the rare diseases community.

Abstract

Scaling transistors into the nanometer regime has resulted in a dramatic increase in MOS leakage (i.e., off-state) current. Threshold voltages of transistors have scaled to maintain performance at reduced power supply voltages. Leakage current has become a major portion of the total power consumption, and in many scaled technologies leakage contributes 30-50% of the overall power consumption under nominal operating conditions. Leakage is important in a variety of different contexts. For example, in desktop applications, active leakage power (i.e., leakage power when the processor is computing) is becoming significant compared to switching power. In battery operated systems, standby leakage (i.e., leakage when the processor clock is turned off) dominates as energy is drawn over long idle periods. Increased transistor leakages not only impact the overall power consumed by a CMOS system, but also reduce the margins available for design due to the strong relationship between process variation and leakage power. It is essential for circuit and system designers to understand the components of leakage, sensitivity of leakage to different design parameters, and leakage mitigation techniques in nanometer technologies. This book provides an in-depth treatment of these issues for researchers and product designers.

Abstract

Brain injury remains one of the most dreaded complications of cardiac surgery. The range of injury is broad; while a stroke may be easily defined and diagnosed, more subtle injuries most definitely occur. There remains debate as to how these are diagnosed and there is a lack of standard definitions allowing inter-study comparison.
